I agree that it's quite a departure for Disney to not include the appetizer for a package, but I do not see what the question is.
The CP package is just that - a package. You are free to order the most expensive items or the least expensive items. It's up to the individual. The tax and gratuity for the meal is based on the flat package price, regardless of how you are paying for that package, either out of pocket or using a meal entitlement on the DDP.
The person paying $53 for one meal gets an appetizer, the person on the DDP paying $37.99 for a day of dining doesn't. The person on the DDP will still get a CS meal and a snack for the price that they've paid for that day. The person paying $53 out of pocket will not get the additional meal or snack.
I think the person using the DDP is still getting a VERY good deal using one credit for a $54 meal. The cost is on par with what a Signature Dining Experience would cost, and in many cases is more. At any 2TS location, you don't get an appetizer, don't get priority seating for your show, and pay much closer to $50 for that meal considering you're using two days worth of credits.
The person paying with their DDP will have to tip 'more' than they would have if they'd paid OOP for the items that they are ordering. That amounts to about $1.30 per person assuming that the person orders the most expensive items and your total OOP would be $7 less than the package cost.
