Could someone please correct me if I am wrong?

DD has her first cotillion in two weeks. We have to turn in the personal information that will be used to introduce her when DH escorts her in. I think because it is a formal occasion the correct form will be: Dark Dancer, daughter of Mr. and Mrs. John Q. Public. versus: Dark Dancer, daughter of John and Mary Public. DH thinks it does not matter. Who is correct?
Also, since her father is her escort, how would you word her "date" for the dance to follow. I do not want to use the word date so I thought of saying "Dark Dancer's friend, Insert his name, will accompany her to the dance".
I hope I have stated my question clearly enough.
My dearest friend who we sadly lost to cancer of the brain was my Miss Manners expert. Can someone help me?

Yes, DH and I are still married. The reason I am unsure of how to address the dance date is the girls will be walked in by their fathers and the wording of that part is: Dark Dancer, daughter of Mr. and Mrs. John Q Public, is escorted by her father. Since the word escort is used there, would you use it again as in, Dark Dancer, daughter of Mr. and Mrs. John Q Public, is escorted by her father. Her escort for the dance is insert name?

Im not sure if you are still married to her father but if so then yes you'd word it DD is the daughter of Mr and Mrs John Q

I would say that she is being escorted by baby louie or whomever

I like the flow of Mr. and Mrs. John Q. Public. it sounds much nicer to me. As for her date I think you are ok there too.

I am not an expert, but I feel that even though etiquette probably says to list the parents as "MR and Mrs. John Smith", I have always thought that including the mother's first name is better. Those old friends of yours may not know your husband's name, but may recognize your name attached to his. Also, daughter's date should be listed as "Suzie will be escorted by Michael Johnson".
