Contract question for those who've rented out their points

Can anyone post for me a sample "contract" between you and a renter? I want one that protects me and the renter as much as possible, as I want the renter to feel comfortable renting from me.

What details should definitly be included?

How do you handle the $ thing, and if I have an April use year, what is the best month to rent my points out for use during (for the renter's and my benefits)?

Anything else I should know? I looked at the FAQ's and it didn't have anything in it that I wasn't already aware of...
