Combo meals without Dining Plan

If not on the DDP, are drinks included in a combo meal (example: burger, fries, drink, no dessert) or would I have to purchase it separately?

They are not included (you can get a cup of ice water for free).

When you are ordering an entree or main dish from a menu you'll receive everything that is included in the description of the item (e.g. if a menu says "burger with fries" that is what is included). It is rare for a beverage to be included with the meal or entree at table and counter service restaurants where you order from a menu or menu board. A cup of ice water is free, but bottled water is not. A fountain beverage is included at buffets, all you care to eat family style venues, CRT, and some set price meals and dining events - the server will let you know.
