Does anyone know if a child (who is paying adult prices on the dining plan - 12 year old) who orders an entree from the child's menu can still have an appetizer and/or dessert from the adult menu?

As far as Disney is concerned, your 12 year old is an adult so he or she can order anything from the adult menu. As far as ordering off the children's menu, I can't imagine that they wouldn't allow it.
 
My experience has been that anyone can order off of any menu for anything at any time. So yes, your child can have an adult appetizer, a child's entree, and an adult dessert.

My husband has said there are two signs you'll never see at Disney World - "if you break it, you buy it" and "no substitutions please".
