Cast Member Praise Question

I realize that a lot of people make or print cast member appreciation cards, but how exactly do I file a formal notice of praise in person? I've tried to look for it, but I'm getting mixed messages. Is there a good place to do this while you're still in the individual parks, like before I leave? Or from my resort? And what should I ask for to get the right form?

If at a Park you can stop by Guest Relations on the way out.

In any event, make sure you include what was on the nametag, the location (as close as possible) and the time and date. They can see the actual name that can be matched up with this information. And a comment will go down, through management channels, to the direct supervisor of the person. And, if it is a favorable comment it will usually be publically read at the pre-shift team meeting.
