Ever since getting a candy apple at a carnival as a kid that felt like I could break my teeth biting into it, I've preferred caramel apples - no extra toppings - just the caramel. Funny how one odd experience can change our preferences.
I love them both, caramel more so that is my vote. However, I love my fillings more. My DMom and I joke that we put our dentist's son through law school. It was funny until the dentist said he is now saving for his grandson based off of our work.
He even told me once that he has no idea how perfectly healthy teeth can decay so rapidly in 6 months.

We just threw away 3 caramel apples. My kids "made" 2 of them on Sunday at a birthday party, and someone gave us one... we offered them to our neighbor but they didn't want them either.
OTOH, my wife is recovering from shoulder surgery, and someone sent us 5 of those super fancy apples... the kind that cost $10 or so each, and my family ate all 5.... Though I must say, that while they were very good, I still would rather have had a candy apple.
Marshall's Fudge Shop in Mackinac City, MI makes the best caramel apples I have ever had. Big Granny Smith apples dipped in their sweet cream caramel...Oh.My.Gosh! You can have them like that or get more "stuff" on them. I love the caramel apple that has white and dark chocolate splattered on it (not sure of the right word for that..."drizzled" maybe???). It's enough chocolate to give it a chocolate kick, but it doesn't distract from the caramel. DH likes either cashew halves or Oreo cookie crumbles on his!
We get these every September when we visit Mackinac Island.....Yummmmmmmmmmmmmmmmmmmm!!!
I love Candy Apples but now I am allergic to apple so I can't eat them very often. Once a year on the the way home from the fair. And I have to eat them with a Bendryl chaser.
