does anyone know if you can reserve a wheelchair or ecv in advance by phone or at the resort before you get to the themepark? my grandma needs one for our march trip.

I don't think so, but you can get one delivered from an offsite company for the length of your trip, that way she'd have it for use around the resort or any other off-site places you may with to visit. One mentioned most often on the disABILITIES board is http://www.walkermobility.com/ there are also others mentioned, if you want to head over there and check it out http://www.disboards.com/forumdisplay.php?s=&daysprune=30&f=20 usually they are a little cheaper than the daily rentals at Disney.

The Wheelchairs and EVCs at the parks are on a first come basis. The resorts have a few wheelchairs but they are also on a first come basis.
I would also suggest that because of mobility issues she should rent an ECV
(Electric scooter) from an offsite company.
I have limited mobilty and have rented mine from Care Medical (800-741-2282) and Walker Mobility (888-726-6837) in the past. I would recommend either one. The cost is about $30 a day. The ones at the parks cost $35 a day and are bigger and have 4 wheels kinda like a golf cart. They are much harder to steer and to control. If she rents from an offsite rental they will deliver the EVC to bell services at the resort. She can use it to go to the pool, and get around the resort as well as the bus stop.
Almost all the Disney buses are accessible. I love using my EVC it gives me more independence and helps keep my pain to a minimum. If she decides to rent an EVC (which I highly recommend) she can practice using one at a local store such as Wal Mart or Target. Those are a little bigger than the rental ones but the practice will help.

I don't think you can reserve them ahead of time (though I could be wrong), but a better alternative might be to rent one from an off-site medical provider for the length of your trip.

There are several providers in teh WDW area who will deliver a wheelchair or ECV directly to your resort, show you how to operate it and recharge it properly, and pick it up at the end of your trip. This will allow grandma to have her own chair or scooter for the week and never have to worry about the theme parks running out of them before whe gets there.

All WDW transportation (busses, boats, and monorails) is equipped to handle wheelchairs and ECVs with no trouble at all, so getting from place to place will be a snap.
