birthday fun card procedures

I'm starting to plan my November birthday trip and right now I'm trying to figure out what days I'm going. One of the reasons for this trip is to take advantage of the birthday promotion; I want the fun card Anyway, my birthday lands on a Sunday this year. My boyfriend and I do not like to go to the parks on weekends so I'm thinking we could drive down there on my birthday and go to the parks Mon, Tues, and Wed. So my questions are:
1. Do you have to use the fun card on your birthday?
2. Do you have to show proof of other entry media (park hopper/AP) in order to get the card? Or is it just an option you can pick just 'cause you want it instead?
3. Do you have to enter the park that day to get your fun card? I know you can get it at the ticket booths outside, but do you still have to use your park admission that day to be eligible for the fun card?

Nope! It doesn't expire!

I think you need to show proof. From what I understand the birthday alternatives are only available if you have an AP or have already purchased a 3 (? if not 3, it's 2) day or more Park Hopper ticket.

This is a good question. You can get your fun card outside of the gates (guest services, I believe). So you may NOT have to use your park admission.

Just sneaking in here to ask if they've stopped making changes to where the card can be used? I gave up trying to keep track but would be interested to know where the cards can be used now.
