I can only speak to our experiences on our recent trip. We tried to eat at counter service places that were already rated highly though some were much better than others.
Best Meals:
DHS - Starring Rolls (Not a high selection but sandwiches and their cupcakes were very good.)
Epcot - Boulanerie Patisserie (Didn't have sandwiches, but pastries were very good.)
Epcot - Kringla Bakeri og Kafe (Ham and apple sandwich, roast beef sandwich, school bread, cream horn. Almond pretzel. All tasty.)
Decent Meals:
Epcot - Sunshine Seasons (Good but not excellent)
AK - Yak and Yeti Counter Serve (Decent, nothing special.)
Worst Meals:
Coronado Springs Resort - Pepper Market (Breakfast decent, Mexican dinner was bad)
MK - Peco's Bills (Toppings were great, hamburgers were way too dry, desserts cheap quality)
Again, we only ate at the ones that were listed as being good. There has to be a lot worse ones out there!
