1. Not usually.
2. Depends on the CMs who happen to be staffing the MK gates on the day you are there.
No set "rules."
3. It's WDW. Nothing ever happens exactly the same way every day at WDW.

I was wondering this also (and it looks like we'll be there the same time, old feller!). So for an 8am ADR at BOG -- we should be through bag check and towards the LEFT MK tunnel (ADR entrance) at 7:40-7:45am?

On our September trip we had two 8AM BOG reservations. One morning we were all let in at 7:40ish and one morning not until 7:50.

That's a good plan.
(Although they have been known to switch the early entrance side.
Just check when you arrive.)
