Anyone use a photo pass?

I am in need of the permission slip from photo pass to print pictures at Walmart, Target. I shared a Photo Pass on here last Aug/Sept and have lost the permission slip.

If someone could email me 1 I would be grateful.

It seems you have been emailed the release but I thought I would note that since you must have gotten a copy of the Photo Pass CD , the release is easily acessible from the cd for printing.
