Am I Overlooking It Or What???

I have searched the Passporter website several times now and I cannot find the "Resort Room Request" form. I used it for our last trip which was in March, but now I cannot find it. Is it gone or am I "losing it" sort of speak!!! Can someone please help me with this dilemma?? Thanks so much!!

I have tried to get to the fax form also. I followed a link that was posted recently and the Passporter site said that they no longer recommend sending a fax, but calling instead and give a phone script. I really was interested in the fax.
