Ala Carte and add on's

How does Ala Carte restaurant work with the DDP? Do you pay extra for your sides or are the still included? Also, some restaurants have option for adding item for extra cost you still pay the additioanl cost right?

Sides listed as coming with the entree are included. If the entree is listed as "steak with mashed potatoes" then the mashed potatoes are included.

Sides listed separately are not included. You'd have to pay for those.
