ADR cards

I have been trying to create some ADR cards for our upcoming trip, but I can not find even a logo to download. Can someone help?

Here is what I am hoping to do, put a logo with restaurant name in one corner then have lines to put the ADR number, time, date etc Even a place for me to put directions if needed at the bottom, all on a index size or smaller card size.

I have time, just not the ability to find the logo.

Any help will be apprecated very much.

Thank you, I knew I had seen something like I was wanting, but could not remember where.

Had most of the ADR's, only Shutters, Tony's Town Square, Rose & Crown and Capn Jack's needed.

Do you think you could do those for me???? That would be great if you could.

Your work is wonderful, I have tried and I lack the skills.
