We are a family of 2 adults, on 4 year old, and a two year old. It looks like we might be having twin 1 year-olds join us. Am I going to have to pay extra to add them to the reservation?

We are staying in a Royal Room at POR if that makes a difference.

That combination makes you a party of 5 + 1 infant*. You can only stay in a room with an occupancy of 5 (which you might have). You won't have to pay extra though since no one is aged 18.

*only 1 of the kids under age 3 does not count towards your room occupancy, the other 2 will count.

You won't be able to stay in RR at POR with that many children. If you have more than 1 child under 3, Disney counts them as occupants. You would have to look at staying in an AB room.
