Adding/Removing guest from reservation

I am a foster parent, and would like to bring my foster son as well as my other children on our next cruise. Unfortunately, whether I will still have my foster son living with us when we travel is something I can't foresee right now. I am also trying to negotiate with the department of children and family services on being allowed to take him out of the country for a vacation. All of this makes for some uncertainty whether he will be travelling with us. So here is my question. If I include him in our reservation and if he won't be living with us anymore and therefore can't travel with us, how hard is it to make a change to the reservation to remove his name and get a refund? Would the better choice be to not include him in the reservation and add him later, or is that the more difficult option? Anybody have any experience with this?
 
Don't forget, you would also need a birth certificate/passport for this child (I assume you already have required documentation for getting medical treatment). You might want to start whatever paperwork for this now, so that you aren't scrambling to get a passport at the last minute.
 
We will make sure to add him to our reservation when we make it. When it comes to documentation, I am working on it with the state right now so that we can take him with us. This is the problem, I am not sure they will let us take him out of the country. We take him to Disneyland all the time, but this is out of the country so we are unsure if we will be able to do this. I can't go on a cruise and enjoy myself if he can't come, so if we don't get the documents we may just do 7 days in WDW. The kids will love that, only my feet will complain.
