? About the Bahamas Immigration forms

On part 2, I booked thru DU. Do I still have to put that as well as their city? (Whcih I don't know)

Yes, you should enter the city for DU on the immigration form. Wasn't anything (letter, note) from DU included with your documents? It should have their address. If not, check their website (there are many links here as they sponsor this website). Or call or e-mail them.

My docs came directly from Disney. I'm sure somewhere on DU's web site it must be listed. I'll just keep looking!

I had the same question, so I e-mailed my DU agent. On part 2 you list the travel agency name as Dreams Unlimited and the city is Blackwood, NJ. Let the countdown continue.......
