Can adults order from the children's menu? Any place this is NOT allowed, if so?

Yes adults can order from child's menu, as far as where it's not allowed, it's a moving target. Best bet is to just ask. I do it all the time since I'm not a big eater.

Liberty Tree Tavern dinner and Ohana breakfast are family style as well.

Any quick service, you will have no problem ordering a kids meal...just like ordering a happy meal at McDonald's...the cashier doesn't know or care if you have a kid waiting for it or not. In table service restaurants, it's hit or miss, depending on the server. IME, you have better luck at the restaurants actually operated by Disney and not a third party. The latter tend to be bigger sticklers for the "rules".

The accepted ordering at an All You Can Eat place like 'Ohana is that adults pay adult prices and 9 and under pay child's prices. This is across the buffet/all you care to eat genre at Disney, or the real world. At regular QS one can order as they desire. Adults with kid meals, kids with adult meals, etc. At TS it will differ. Disney owned may compromise in bring an adult sized portion of a kid meal while charging and adult price. Privately owned establishments may not, at their discretion.

Family style meals will charge based on age, not what a person eats. They will probably bring items that are palatable for that person, if they usually provide it for kids. Definitely call, let them know, and ask about alternatives if that's the case.

Family style meals include 'Ohana, Garden Grill, Liberty Tree Tavern (dinner only), Hoop De Doo, Spirit of Aloha Luau, and the family platter at Whispering Canyon Cafe.

You can order from the kids' menu at any Quick Service restaurant because there's no way of knowing who you are ordering for. At Table Service restaurants most will allow anyone to order from the children's menu, but once in a while someone will post that a server or restaurant followed the "Ages 3 - 9" that is printed on the menu so it is best to ask first.

You will be charged according to age at buffets, family style meals, fixed price meals (e.g. CRT, Akershus), Dining Packages (e.g. Fantasmic, Candlelight, Jungle Book), special events (e.g. fireworks parties, F&W events), the 3 dinner shows (Hoop De Doo, Spirit of Aloha Luau, Mickey's Backyard Barbecue) and the Family Platter at Whispering Canyon Cafe.
