In the Disney parks menus and signs are in Japanese and English. CMs speak Japanese for all communication, but most can speak some English. Some are better than others. The Japanese learn English in school, but don't have much opportunity to speak it and in many cases may be embarrased to try because it is not very good. I really didn't have any problems though. English is on most menus and such in Tokyo itself. Sometimes it may be hard to find a particular place because very few of the signs are in English. Most hotel rooms in Japan are small. From what I saw on the websites for even the Disney resort hotels, the standard rooms are smaller than what we have in the states. You really have to pay a premium for a larger room.
