It is from the Pirates of the Caribbean Quest that you do on the VMK Insider Tour at Disneyland or the Magic Kingdom (Walt Disney World). You get a card for each of the six questions answered correctly on the quest and the room card for answering all correctly.

I don't know if this quest will continue at the Magic Kingdom after the Haunted Mansion reopens next week or at Disneyland once the Haunted Mansion comes out of its Nightmare Before Christmas guise.
