Drink with meal on DDP

I know that the meals include a drink, [non alcohol] and most menus list soda, milk, ice tea as drinks available and I realize there may be others,
of this list though the common soft drinks, are refills at the meal included or it is one drink and then they charge for refills?
I know many other places, non Disney, have free refills for soda or Ice tea but Milk is a per glass charge etc.
How does this work on the DDP?

My understanding is:

CS: no drink refills

TS: drink refills excepting most specialty smoothie-type drinks

My kids do not drink non-organic milk at restaurants so not sure about milk specifically for kids or adults, but they have gotten free juice refills at Disney TS places in the past while not on the plan.
