Generally, no, you cannot substitute an appetizer for an entree when on the DDP. You can ask, but I've not heard of it being allowed very often. The third party venues won't allow it because they won't be reimbursed by Disney for the appetizer.
It totally depends on the restaurant and random things like the phase of the moon.
All you can do is ask when you are there, but don't plan 100% on being able to do it (even at places someone else was able to do it). Have a backup plan.

Exactly! You can even do this if you get an entree that is not DP worthy. I mean cheap. Just tell the server which meals will be DP. Or dont use the DP at all!! Figure out what works best for your family!
The answer that most fits the question is "maybe" because there are some restaurants that won't allow it. Usually these will be 3rd party restaurants (not operated by Disney.) Most of these can be found in Epcot's World Showcase or Downtown Disney.
