What items are not included in the DDP? I've read that some items may required a surcharge.

Everything is included, but there is like one steak that is over the amount allowed (I think it is 27.99). If you order that, you simply pay the difference. Their food is really good, although I did not care for our server. The pumpkin ravioli is yummy!
