Where to get Medical Release - taking my nephew

I am planning WAY ahead. I am taking my 9 year old nephew to Disney in April 2008. The last time just the two of us went was January 2004. On that trip I typed up a medical release and had my sister sign it and get it notarized, so in case I needed to get medical attention for my nephew, I wouldn't have any problems.

I could have sworn I saved the form on my computer, and maybe I did, but now I have a new computer, and can't find it. I thought I found a sample of the form on these boards. Anyone have any ideas?

I got mine from someone off of this board that either works in a doctors office or husband is a doctor.

Here is what mine says:

We hereby authorize John or Jane Doe the ability to consent for medical treatment for our children Elmo (11/10/00) and Prairie Dawn (9/22/03). This authority is to include such as examination, imaging studies (x-rays, CT scans, MRI's), blood and other tissue tests, prescription and administration of medications, emergency treatment, inpatient admission, inpatient hospital treatment, casting/splingting, etc.

In the event that none of the above stated individuals or parents can be contacted due consent will be given to Bert and Ernistine Muppet (867-5309).

Both Parents sign
