What is a GAC card

I am reading about a GAC Card. What is it? I am going to have to use scooter due to severe leg problems and not going to be able to walk or stand long periods of time. Do I need a doctors note to get one and if so is it better written on letterhead or a prescription pad paper? Sorry,I am not up on abbreviations such as GAC! Thank you
