Mine is fairly simple.

Gas
Road Food
Hotels on road
Hotel at destination
Food
Tickets/Entertainment
Souvenirs

It depends on the destination.

If it's WDW, the budget is:

Airfare
Hotel
Food
Tickets

And that's really about it other than incidentals. We don't leave the property and we don't buy a lot else. If I'm going to pay a fortune to stay somewhere, I'm going to get my money's worth if it kills me. :rotfl:

Our typical vacation involves renting a house within a couple hours of here, so:
Rent
Groceries
Eating out
Gas/tolls (not a major expense)
Any entertainment

We don't take a lot of vacations, certainly not one every year. So I tend not to go overboard accounting for every little thing because if we spend a little more, I'm good with it.

This, except we don't really budget for souvenirs as we don't spend much on those. Also, we have to add the kennel for the dog.

Airfare
Resort Package
Tickets
Parking (We're parking off site by the airport)
Souvenirs
Special events (Pirate Cruise, Pirate makeovers etc)

Gas

Destination Lodging
Road trip Lodging
Groceries (we always have a condo/townhouse at the destination)
Destination Dining Out
Road Trip Dining Out
Special Attraction Admission/Tours (ie Disney tickets, national park entrance and tour fees, hot springs admission, whatever special things we are doing on that trip)
Souvenirs

Lodging
Airfare
Car/other transport like rail tickets & buses
Gas
Food & Drink
Excursions/Activities/Admissions
Insurance
Pet boarding
Shuttle to the airport & back home
Clothing (if needed prior to leaving)
Souveniers

Our air and hotel are almost always free (I love credit card points!), so our trips are usually:

airport transportation (home/airport and airport/hotel)
gas (if we rent a car or if it's a road trip)
car rental (if needed)
food/drinks for room
restaurants
entertainment/attractions/theme park tickets etc.
souvenirs
