I ordered the Ariel membership WDCC figurine a few weeks ago from the Disney Store. When it arrived, it came with an order / membership form. Do I need to fill this out to get the Sketches magazine, membership card, and catalog, or will I automatically get those things?
 
I have never subscribed through the Disney Store.
However, every time I pay my membership, I get the figurine, and at some point, I get my Sketches. Every year, it takes a lot of phone calls, because in Europe they mess up big time, but eventually, I get them.

Usually, the only order forms I get are the ones for the members only figurines

Sorry I'm probably not much help because our experience was this:

First signed up with the society last year, whilst in FL (in a store at MGM). We filled out all the paperwork there and took our Belle membership figurine away with us.

We have been sent sketches (to the UK) ever since with no problem.

My understanding is that we have to reenrol on our year anniversary by again filling out the paperwork (I'm guessing it will be posted to us when our fee is due again) and we will then receive the figure of Ariel.

Did you pay the membership fee when you bought Ariel? Did they take your address down? If not then I guess you need to fill out the paperwork it came with or you won't get sketches. If you've already given them your details perhaps this is just a duplicate copy of the paperwork?

Since I ordered it online, and it was shipped to my house, they have my adress, phone number, etc. And yes, I already paid the $50. Thanks for both your replies, but since neither of you ordered through the Disney store website, I'm still not sure on weather or not I have to fill out the paperwork.

Also, at the top of the paper it has dealer information at the top, and then membership info at the bottom. It says all dealer info must be filled in, but what's that? So I really want to know if I have to worry about this at all.
 
My best guess would be - call the Disney store :) or perhaps email? I'm sure they would be best to help with your specific circumstance.

At the end of your one-year membership, they send you an invitation card to go to your dealer and buy a new membership. They have never sent me the paperwork just like that, only a little card to tell me it's time to pay again. I then go to my dealer, she fills out the forms, I pay, and I get my figurine!
 
If you have the original form that says "Dealer" on it then I would say, yes, for the membership to be completed you need to fill it out and submit it.

I would skip the dealer info, complete the rest of the form and send it in.
