We were on the 4/23 magic and our friend booked the renewal vow as a surprise for his wife (he even packed the dress she wore a few years back without her knowing!). I spoke with Katarina, who was coordinating the event that day. She told me vow renewals can be as small or as elaborate as one would want. They usually do about 2 vow renewals on a 7 day cruise. My friend had his family (hubby, wife, teen) and his brother with his wife and 2 teens and us (2 adults 2 kids). So it was a small gathering. The ceremony took place at Sessions, usually 4:30 & 5:30pm. They arrange a small walkway to make it seem like an aisle, there is a piano player, when you get to the end of the walk, the captain is waiting. The ceremony takes place and there is a cake & champagne is poured for whoever is present. I am not sure if you can choose another part of the ship to have it in, I am sure that if it is just the 2 of you it would not be a problem. They give a special "honeymoon" gift - lanyard with pins I think, a certificate for the vow renewal, cake topper, wedding pin and the dinner at PALO for the couple. It was really nice.
