I haven't used them personally since I didn't use travel insurance at all on my last cruise, but my DIL's have. DMIL has several health complications and needs to have travel insurance whenever she travels. They have always been very good to her and she has needed to get reimbursed on her last two cruises for Dr visits. This time was just for migraine medicine and I've not heard how they have done since it's been less than a month, but last time was a full blown clinic visit in Mexico.

We use USAA for ALL of our insurance, including travel. Thankfully, we have never needed to file a claim for travel insurance, so I don't have any experience to share. I have "shopped around" for TI & found USAA to be the least expensive option for us.

Yes, i have used them twice for our cruises. They are great & have the best prices. I just booked a Disney cruise for Oct. 07 but used another TA, only because they were offering a ship board credit. You can't go wrong with USAA.

You can still use USAA travel insurance even if you booked the cruise through someone else. We used a different TA on our cruise in January and DMIL used a different TA on her previous cruise, but used USAA travel insurance both times. You do need to make sure you get the insurance within 10 days of making the cruise reservation to cover "pre-existing conditions" though. We know that one for absolutely certainty.

And yes, USAA is amazing to deal with all the time. I love them.
