Trip Itinerary Template?

Hi everyone. I was wondering if anyone has a trip itinerary template they would like to share? We are Disney Veteran's but now traveling with our DD who is 2, I like to have everything "planned" and laid out. I normally can create things, but for some reason I am having a major brain fart! Anyone want to share?

I have another DISigners planning pages in my photobucket
look under Dolly's Creations--I do not make any changes to her DISigns and she is not currently DISigning.
