Tipping Guidelines

I see lots of talk on the boards about tipping "policies". My personal policy has always been that a server earns a tip. I have left restaurants without leaving a tip, if the server was rude and inattentive. We are not hard to please, but there have been times (very few) where leaving a tip would be completely inappropriate. I have also left very generous tips for exceptional service.
Is tipping in WDW really that different than the "real" world?

Please don't jump all over me. I expect that I will be leaving the appropriate tip at each restaurant we eat at.
