Those great Mickey heads with pictures in them

Help!!!
People make great signs and t-shirt designs with pictures inside of Mickey heads that look like bubbles. I don't know how to quote or I would pull some from other posts to show you. I've seen resort t-shirt designs and there are several on the cruise forums thread about decorating your doors.
I can't figure out how to do this. It's driving me crazy. It's become a quest.
I have photoshop elements 4.0. Can anybody help me?

Thanks for the idea. I've looked at the t-shirt designs before, but I couldn't remember where I'd seen them.
