Teachers-some advice please

Except when the TEACHER misplaced the papers.

Sorry, by her attitude and disorganization I wouldn't classify her as an "excellent" teacher, sorry. I too would ask for weekly updates on missing assignments and KEEP everything from now on. My guess is you will get one, maybe two updates and you won't see one for the rest of the year unless you are the one to call her and ask for one.
 
Well hopefuly after this meeting the problem will stop and the teacher will be more on top of things.
 
You had multiple examples showing the teacher had missed recording work done. While anybody can (and will) make a mistake, I think that's excessive.

When I was still in the classroom, I had an "in" box and an "out" box on my desk. Students were responsible for putting their work in the "in" box. I only graded what was in there. They knew not to "just leave it on my desk" due to how easily papers can be lost or misplaced (and were reminded of that regularly).

The papers only went straight from that box (each set of papers was clipped together when they went in the box) to the surface of my desk to be graded/recorded, and then straight into the "out" box (sets clipped together again) to be distributed to the class.

The teacher that you're dealing with definitely needs to work on those wonderful organizational skills that she's crediting herself with if you found that many errors on your own (and that's just one student in her class).

the majority of "missing" were items from sporadic day missed due to illness (in which case she was told what she had missed but the teacher forgot to tell her about/provide her with handouts that done in class (none of the items were homework), or items when the she was out (planned and approved by the teacher (who is also the principal) the spring prior-we had gotten all of the work done prior to leaving then the teacher decided while we were gone to change which math pages were done, and the same in other subjects.

teacher does have an official in box where the kids submit their work, but i have observed her as well as ta and parent volunteers collect work from the kids and put them inside the instructional book they are using at the time, put them on a work table, put them in a pile on teachers desk...
