You can enjoy this Dole Food option if you
become the first place winner in the 2005 "Creative
5 A Day Teacher of the Year" initiative. Any
elementary school teacher who creatively uses 5 a day
activities in the curriculum to motivate their students
to eat more fruits and vegetables and become more
physically active is invited to apply.
Access the official entry form at Dole's website at
http://www.dole5aday.com. Your entry must be
received by March 18, 2005, and winners will be
selected by April 15, 2005. Awards ceremonies will
be held in May 2005.
The first place winner picks either a trip to
Disneyland in California or Disney World in Orlando,
Florida for four people for five days and four nights,
including a Saturday night stay; roundtrip coach
airfare for four; double occupancy hotel accommodations
(2) rooms; and $500 spending money. The
trip must be completed by May 31, 2006 and is
valued at $5,000. As first place winner, you will
also be honored at a luncheon party limited to 75
guests planned and executed by Dole 5 A Day
Program.
Second and third place winners will be honored
at their schools in an awards ceremony where teachers
will receive framed certificates and students 5 A Day
gifts.
