First off I would like to say that honestly, both my wife and I are educated people. :teeth:

For such a simple form, DVC has made it very difficult to understand... Or at least *I* don't understand it.

I have a call into the number, but I need to get the thing in the mail today and can't wait for a return call.

Okay, top section:
Sponsors- I signed, dated, and printed my name. Then my wife did the same right under that... Simple enough?

Bottom section:
Address, city, and state... No problem with that.
Name- I put Karl & Maja Fxxxxx. I guess that's right..

Now, the dashes for the tapestry characters:
We put "Karl & Maja", since our last name won't fit.
After reading the other tapestry thread, and part of the pamphlet, it sort of seems as though they just want our "family name"?? Is that our last name they want? If so, why did they have us put that in three other places on the same form?

The whole thing seems simple enough, but I have the feeling that something's amiss
Any other reasonably intelligent Members here as confused as I am??

I read the request for the family name to mean last-name. I didn't bother with first names or initials or any other variations on the theme. Since they limited the space to 16 characters, it's unlikely that DVC intended the inclusion of anything other than the last-name of the sponsor family.

That was my take anyway. There just isn't room for most peoples' first & last names in a mere 16 spaces.

with dvc asking us for our address, town and state, im wondering if they are going to stich on the quilt example: The Kaxxxxx Family
Town, State.

or just The Kaxxxxx Family and not where we are from, why would dvc ask us our address

what if we put Kaxxxxx of STate. and then dvc puts our town and state:confused:

yes, its all confusing and im running out of time to send ours back.:headache:

But what if your last name is a common name like Smith, then there maybe 10 squares that say Smith. it would have been nice if dvc gave us an example and what it may look like.

Yeah, I guess I'm going to cross out our first names and put our last name there.
Perhaps they need the name to be used for the tapestry on a different line, since some couples use two different last names and such.

I guess all the creativity in the other thread is out the window.. :smokin:

As I recall, the insert that came with the form did mention that DVC would call to confirm your information. So there should be a chance to change the info then if need be.

An example would have been nice, especially if they planned on not answering the phone!:rotfl:

And MG,

Im confused too! And Im pretty darn smart when I want to be!:wizard:

My last name is hypenated, but Im willing to cut it down if we do something creative. If not I want the whole last name on there:confused:

I have called that number 4 times left all my contact numbers and POOF:wizard:

NOTHING NOTTA ZIP ZELCH ZERO:scared1:

SO I put what I would like which was 1 letter more and a note saying you never called me so this is what I want

Exactly! Which is why I have a different last name than my husband. I put our first initial and last names on the form.

We are K xxxx & D Smith. It will look stupid if they morpth that into "the K xxxx and D Smith Family" I think.

I thought I remembered reading that DVC will call to confirm the inscription once they begin to process the forms.

I wouldnt hold my breath. With this timetable, they must be engraving our names on tombstones:hourglass
