Yes I know we have discussed this many times but the search engine won't work for me AGAIN.
I have always taken my own daughter to WDW/ and abroad with her stepdad by writing a letter from him, getting him to sign it and have NEVER had an issue. Never had them notarized either... however, I am going a few days earlier this trip with my 3 younger children and a friend. My husband will be joining us and bringing my daughter (his stepdaughter) and his nephew along with my mom. (Who has the same last name as my daughter).
Can someone give me the link for the appropriate example of an official letter to cross border with a child please?
Also how/where/who do I get to notarize a letter? I have 4 kids (oldest in a wheelchair and severely disabled) so if I don't have to lug them all out, I'd rather not. Will be sooo much easier when the baby is mobile and a bit more independent.
Trip is NOT until September but we are in need of doing this sooner rather than later.
Thanks for all help in advance.

Never have done this myself (but will be next year), so I looked this up for you on TripAdvisor. Sample letter included at the bottom.

Travelling with Children
If a child is traveling with one parent, the border services official may require a letter of permission from the other parent or documentation that demonstrates that the lone parent is the child's sole guardian. If a child is traveling with someone other than his or her guardian, border authorities will want to see a letter of permission from both of the child's parents / guardians. Ideally, the letter should be notarized. You can use this sample letter of consent as a starting point.

Note: It is strongly recommended that children travelling alone or with one parent carry a consent letter for each and every trip abroad. Although anyone can witness/sign these letters, it is advisable to have the consent letter certified, stamped or sealed by an official who has the authority to administer an oath or solemn declaration (i.e., a commissioner for oaths, notary public, lawyer, etc.) so that the validity of the letter will not be questioned. It is up to each official/individual who witnesses such a letter to decide what proof he/she needs to see to be able to witness/sign the letter. Officials should only witness/sign a letter of consent if he/she is convinced that the individual requesting the letter is who he/she claims to be and that adequate proof has been provided.

This consent letter may be required by foreign authorities among other additional country-specific entry requirements. You should contact the representatives of the country or countries to be visited by the child to ensure that you have the most up-to-date information regarding their specific entry requirements as permission to enter another country is the sole prerogative of that country.

We also strongly recommend that you contact the transportation company (airline, train, bus, etc.) in order to observe any additional policies that they might have in place.

Further information is available in the FAQ and Children and Travel sections.

The following sample letter can be amended to meet your specific situation and requirements.

Have travelled several times with DD without my DH. I always have a travel letter drawn up by our lawyer. Our office takes the required travel info. over the phone. My DH goes into the office a couple of days later & signs. Charge is minimal. I have always been asked by customs to show letter both flying & driving over the border.

Minimal cost for peace of mind & ease of travel over the border.

My CAA rep told me that your local MP can draw up a letter for you as well.
She told me that Saturday, so I have not confirmed that.

I took my niece with me in March, both of us have different last names.

Below is a link to the Canadian Governments travel consent letter template. This is the one my sister and brother-in-law completed for when we traveled. My sister had contacted the US Border and they told her that the letter didn't need to be notarized so we didn't get it notarized.

I was required to show the letter when crossing the border into the US, as well as when returning back into Canada.

This is the letter that we used when we took our niece and nephew to WDW a couple of years ago. I saved it as a word file and just filled in the blanks. It looked great too! My sil had it notarized, just in case. We weren't asked to produce it, either coming or going but I was glad to have it just in case.

As a side note, when my sil brought the letter to our local notary he said that since everything looked so good and he didn't actually have to do anything but sign it that he wouldn't charge her for anything. She did bring him a Timmies, though! Aaaahhh, small town living is good for something!;)

Does anyone know if you have to produce the letter at the airport?
I will be crossing in October in my car then flying. I know the border officials require the letter but do airlines?

This is the one we use, and it was the only one recommended to us by the passport office. And I would definitely have it notarized -- in fact, they don't have to accept it at all if it's not.

I used it 2 years ago when traveling alone with my DD, and we had to produce it several times ie. every time we had to show our passports eg. airline check-in, customs pre-clearance.

I have taken many friends over the years and never shown a letter to anyone.

I do get a letter from their parents but have never been asked to see them.

As for my own kids I have never even been asked for a letter from my ex- if you have sole custody you do not need permission to take them.
