T shirt design/ "Gift Certificate" needed

My DH birthday is approaching at the speed of lightning and has he is notoriously difficult to buy for, I have decided to pay for a couple of treats for him for our upcoming vacation. I will create a second thread for the other surprise I am planning.

As part of the vacation, we will spend 10 nights at WDW. He has always been crazy about steam trains so I have decided to book the Disney's The Magic Behind Our Steam Trains Tour for him. I would love to make him a t shirt for this, but I am creatively challenged. I would also love to have some sort of "gift certificate" to give to him on his birthday. His name is Graham. There would need to be some room where I can add the date and time once I have this.Is this something any of you talented people can help with? I would print this out on letter sized paper.
