submitting Special Services form - email alternative known?

I don't know who still uses fax machines! I guess I'll just snail-mail the form in, but it seems like there should be an email address to submit it to by 2016... just in case, thought I would check here and make sure I didn't overlook a way to submit the form other than fax or snail-mail? Thanks! I saw that on the web-site, but it said it was for more information, and wasn't listed on the form as an option for form submission... so I wasn't sure that was a valid way to submit the form. I wonder why they don't just put it on the form??

We've used the email and never had a problem. They typically respond within a few days.

There are "free fax" services that basically fax what you scan and send. Fwiw.

I use the above email when I submit mine and have never had an issue. They reply back within a few days.

I also use it to email if I have questions -- often it is easier than calling. Sometimes they write back, sometimes they phone.
