Submitting a voucher to SW

I posted this on the Canadian board but no replies, so hoped someone on the Transportation board might have some experience with this:

Dh has a $200 voucher from Southwest for going and retrieving his delayed luggage from Buffalo airport, instead of them delivering it to our home.

He just returned from a trip and will be sending this voucher to SW for reimbursement for his flights and I was wondering what I should send along with it, from anyone who as experience with this.

I have the email confirmation from booking the flight, a couple of baggage receipt tags to show he actually went on the flight (if that's necessary) and the voucher.

Is it necessary to send the credit card info or do they have that on file, to send the credit to. The agent at the airport had circled an address on the voucher of where to send it to but there's no other instructions.

Other information from anyone who has sent in a voucher to SW would be greatly appreciated! :)
 
I did this once, but it was a few years ago. I believe I just had to send my flight confirmation and the voucher in (sent certified mail of course!). You might call the customer service number just to double check and make sure nothing has changed. It was pretty quick and easy as I recall! :thumbsup2

Anyone else have an experience/info in submitting a voucher to SW? I might call Customer Service but it's always good to hear from fellow DISers as well!
