My niece's bday is on an EPCOT day and she loves Mexican food so I have lunch at San Angel that day. Do they do anything special for bdays or can I request something to make it special?
The last time we were there and my wife was wearing an "It's my birthday" button, they brought her out dessert. I don't recall which dessert (ice cream or cheesecake) it was, but it was a full size dessert.
Make sure you note the birthday in the reservation. Also remind them at the front desk when you come to the hostess' station. Also tell your server when they order your drinks. Make sure you wear your birthday buttons!
Don't be pushy, just keep it in the conversation and whatever it is that San Angel does, they will do their best to give to you. Sometimes it's on the way out, though, our Chef Mickey's birthday card and such did not come while we were eating, it was almost an afterthought while paying, but we got it on the way out the door for the scrapbook.

Have her double-check the menu before you go. She may THINK she loves Mexican food, but what she really may love is border Tex-Mex. That's our case (good, since we live on the border), but we didn't recognize many of the offerings at San Angel Inn.

If she wears her birthday button, the only guaranteed recognition will be castmembers throughout DW will say "Happy Birthday." At restaurants you can also mention it to the host/hostess and your server, but there is no set policy and it will be up to the server and restaurant whether it will be recognized or not. We've gotten desserts, confetti, champagne at some restaurants to absolutely no recognition at others. Many thousands of visitors are celebrating a special milestone during their visit (e.g. birthday, anniversary, graduation, first trip, family reunion, etc) that there is no way DW can recognize every one of them.
If special recognition is important it is best to plan and pay for it yourself. You can order a podium cake at most Table Service restaurants or for something more elaborate check out the cake thread DisneyWishes posted upthread.
Also, San Angel (and La Hacienda across the walkway) are more traditional Mexican food and not the typical Tex-Mex many are more used to. You can also check the menu for Rick Bayless' Frontera Cocina at Disney Springs.
