You will make any requests at reservation, or you can call reservations and have requests added on later...but try not to be too specific, or make too many demands. Much better to say "first floor preferred" than to say" room 1201". You're much more likely to get your wish fulfilled that way.
From what I understand, the resort itself will not be able to help you. You can try a little dickering at check-in, if you're early enough in arriving, but don't count on too much.
I have been able to call the resort directly and change/add a room request. It is possible to make room requests both through CRO and your resort directly.

I have made my room requests through my reservation. Last year I also faxed my request in as well. I think it was to the CRO. I remember reading that this practice isn't welcome anymore.

My understanding is that faxed requests are pretty much ignored now. Thanks to the internet.
I would put my request in with CRO so it is ON the reservation.
Also, "specfic" room requests... keep in mind that Disney is not throwing guest A out so you can have room X!
I just called central reservations and had it added on. The guy on the phone told me that they don't really accept faxes anymore...that it was best to have it noted on your reservation. He told me that the chances of not getting your request was very slim.
