How far ahead of arrival should 'special' requests be made and what is the best way? We have always called about 4 days ahead and gotten whatever we requested. This time we need a ground floor with ahandicapped bathroom (rails).

You should call member services and have it added to your reservation. DVC has said that members should not call or fax the resort directly. There's no time limit, you can do it when you make the reservation. I think 4 days might be too late, they may have already started blocking rooms.
 
There is no reason not to make your requests when you book your room. Sometimes rooms are assigned up to 10 days before your arrival.

Needing a HA room should already have been requested. There are only a few and when they are gone, they're gone.

The HA room on the first floor was requested at the time I booked the room. We're mainly intered in a location request. We have always called in the past about a week in advance but things change.
 

I just make requests when I make the reservations.

I made reservations at OKW and put in a request to not have to change rooms when we go from a 1 bd to a 2 bd. MS suggested I call OKW directly about a week before my travel date to remind them of the request.
