Resume/CV information

I got an email telling me to fill out a request additional information form, it asked my birthday then said i completed the form so I'm confused since I'm sure they wanted more. My dashboard says Resume/CV information not available now. So is this a problem on their part or did I not fill something out?

This is the additional information form you received after you were accepted, right? If so, your birthday is the only thing that was needed to fill out.
