Restaurant Tipping Checklist

I know I have seen something like this but can't find it:sad1. It looks like a checklist and depending on how points quote the server when u add it up it tells you how much to tip the server... Anyone knows where I can find that checklist?

Not sure what this is, but why don't you just take 15-20% of the total bill?
IMHO, I would think that focusing your meal on filling out a checklist would take away from enjoying the meal and the company you are with-?
