Most of the menu boards will list what comes with the meal. Also, several of the boards have the actual photos of the meals. Most adult meals come with fries or apples, but there are a few different options especially at the resorts.
When you read the description of the item on the menu you will get whatever is in that description (e.g. Hamburger, comes with choice of chips or cole slaw). If there's nothing in the description other than the main item, that is all that's included.
Not necessarily. My wife got a fruit plate at Flame Tree years ago which listed nothing and they asked her what side she wanted. Of course, policies may have changed. I find with Disney it never hurts to ask as sometimes their rules can be confusing.

My point was the menu will explain what automatically comes with the entree. If it just says fruit plate you can't expect additional sides. If the CM offers, that's great and of course, you can ask, but the OP wanted to know how you could tell what to expect.
