Questionnaire for group trips?

Does anyone have ideas for a questionnaire that you've used when going to WDW with extended family or groups? I think this is a great idea to get others involved in the planning and to give me an idea what they like and dislike but I'm drawing a blank on topics/questions.

I've used them from time to time and they've worked out well! Usually the questions are:

Here is a list of rides to work into our itnerary for (fill in park here). Cross off those you'd like to skip and add any you think we're missing.

What are your favorite snacks?

What breakfast foods do you prefer?

Are you willing to try new ethnic foods?

Would you like to share your meals with another person to keep costs down?

What is more important to you - touring time, relaxing time, or dining time?

I also include a list of things to pack, just in case! :)
 
I've bought Birmbaum's book and given it to people unfamiliar with WDW and ask them to mark the rides/attractions they are interested in. Then I consolidate that information.

if your group is registered for a magical or grand gathering there is polls available for you & your group that you can make & select with waltdisneyworld.com (disney.go.com) you might want to request some info & see if its for you. there is also special events available to mg/gg including a illuminations dinner at epcot, safari and dinner at ak & breakfast at mk.

we did a large family trip in 2000 for a group of 20, & most of the guests had gone to wdw previously we did a general poll on which resort, which group events & a family newsletter once all our details were confirmed about the group events planned. we also had free days, which i highly recommend just so everyone can relax & enjoy what they want at wdw on their own as well.

best bit of advise when traveling with family, friends &/or group, be willing to compromise.
