Question Regarding 2013 Disney Dining Plans

I do not want to start a debate, I just wish to know how/when/where this is being instituted. On the brochure it states:

An automatic gratuity charge may also be added to your bill for items you order that are not included in the Disney Dining Plan (for example, alcoholic beverages).

Is this only for parties over 6 or everyone?

We were there a couple of weeks ago and are a party of 4 and never had a tip added on. There were the suggestions of 18% and 20%.
