Printable medical chart

Anyone know if any printable ( at home) charts with info that you can fill in and or change on your own? Im really only looking for some sort of chart for us to refer to fir changing needles on one of my daughters insulin pens ( she's T1D)
It's used nightly at bedtime, and we change it every 4 nights unless it begins I hurt when she does the injection. I find it easy to forget the days we need to change....new truce writing it sn in the back of her log book, but still not working real good.
I'm organized, and am looking for clever ideas!

Do you have a smart phone?

For things like this that we do I set my calendar alarm task reminder...I set the task, tell it to repeat every 4 days, it reminds me with a pop up reminder on the day and time it's due. If I complete the task early it's set up to automatically remind me every 7 days (or 4 days in your case) to repeat the task. Just have to remember to check off the task on the app when it's done. All this is synced to my google calendar on my laptop, so if something happens to the phone I still have all the info on my computer.

I've gotten so organized since I started google calendar plus the app on my phone (which has the better task manager built in). Everyone in my house is color coded, I forget days off of school, or grading period end dates (or report card dates!), pay weeks, things like changing the air filters in the house hvac system. I use this calendar for all sorts of things! The air filter reminders are great, all set to go off in the middle of the day on weekends....my dh will be sitting down, my phone will buzz and I declare it's time to get the ladder out, we need to change the air filters!

Sorry to ramble on, just trying to show you what ELSE you could use the same thing for (no I don't work for the calendar app people!)

I just use my smart phone calendar for infusion set changes or I just mark it in my log book someway.
