yes it is mandatory for some cruises so if you refuse the test you would be detained (not on the ship but by the police).
And forcibly tested? I can't imagine that. We're talking about the United States here, right? Detain you for a short time. Maybe try to issue some type of fine. But beyond that I have my doubts.

it is a requirement to re-enter the country so yes, you would eventually need to submit to the test or you would be held at the border. no sane person would refuse the test anyway so I'm sure it's never going to be an issue, especially since vaccines are already required.
 
The boards have rules against discussion on how to circumvent rules and policies... this thread is treading too closely to that for comfort. If you have questions about the legality and enforcement of testing I suggest contacting DCL.
